CPSC provisionally accepts $16.875 million treadmill penalty
The U.S. Consumer Product Safety Commission said Tuesday, August 4, 2026, that Johnson Health Tech Trading agreed to pay a provisionally accepted $16.875 million civil penalty over reporting failures involving Horizon T101-05 treadmills.
The settlement resolves CPSC charges that the company knowingly failed to immediately report a substantial product hazard as required by the Consumer Product Safety Act. It is not a court finding of liability, and the agreement remains subject to public comment.
What regulators say happened
According to the CPSC, Johnson Health Tech Trading received at least 874 reports between March 2018 and October 2022 involving treadmills that unexpectedly accelerated, stopped or changed speed. At least 71 injury reports were included. The related recall record describes bruises, abrasions and two broken bones.
The company made production changes in February 2021 and September 2021, but the CPSC said incidents continued after those changes and that the hazard was not reported promptly to the agency.
The CPSC and the company announced a recall of about 192,000 Horizon T101-05 treadmills in the United States on October 27, 2022. The agency said the machines could unexpectedly accelerate, change speed or stop without user input, creating a fall hazard.
What the settlement requires
In addition to the monetary penalty, the agreement requires Johnson Health Tech Trading and Johnson Health Tech North America to maintain internal controls and procedures designed to comply with the Consumer Product Safety Act.
The companies must appoint a product-safety professional, review and analyze incident and injury data for potential reporting obligations, and submit annual reports on their compliance program, internal controls and audits for three years.
The CPSC provisionally accepted the settlement subject to public comment. The August 4 announcement therefore does not mean the agreement is final.
Which treadmills are affected
The recall, listed as CPSC recall No. 23-026, applies only to Horizon Fitness T101-05 folding treadmills. The model name and serial number are printed on a label on the metal frame near the power switch.
Affected serial numbers begin with TM734 for units made in China or TM486 for units made in Vietnam. Not every Horizon treadmill is included.
The recalled treadmills were sold nationwide at Dick’s Sporting Goods and Johnson Fitness & Wellness stores, as well as through Horizon Fitness, Amazon, Dick’s Sporting Goods and Johnson Fitness websites, from March 2018 through October 2022. The listed price was $600 to $1,000.
What owners should do
Consumers with a matching T101-05 unit and serial prefix should stop using it immediately. The recall remedy is a free USB containing software to repair the treadmill. Owners should contact Horizon Fitness through its recall support page, by phone at 888-223-1045, or by email at retailrecall@johnsonfit.com.
Owners should not sell, donate or give away an affected treadmill before completing the recall remedy. Consumers who experienced an injury or malfunction can report it to the CPSC through SaferProducts.gov.
The settlement and recall address the identified T101-05 defect. The software repair should not be understood as eliminating every possible risk associated with treadmill use. Consumers should follow the manufacturer’s safety instructions after the repair and monitor the CPSC’s recall information for updates.
