Montana Opens $4 Million Rural EMS Equipment Grant Program

Montana has opened a $4 million first-year grant program for emergency medical services agencies located in or serving rural areas of the state. The funding may be used for ambulance and other EMS equipment upgrades, and applications are due August 15, 2026.

The Montana Department of Public Health and Human Services announced the opportunity July 16 through the state’s Rural Health Transformation Program. The money is available for applications, not already distributed payments to agencies or individual residents.

Who can apply

The opportunity is open to EMS agencies located in or serving rural areas of Montana. Eligibility is not automatic. The application guidance says eligibility is based on need, including the age, model or operational status of current equipment, gaps in patient care, the level and type of service provided, and anticipated positive outcomes from the proposed equipment.

DPHHS directs agencies to the Health Resources and Services Administration’s Rural Health Grants Eligibility Analyzer to check whether they are classified as a rural health provider. Agencies should also review the state’s application criteria and document the rural communities they serve.

What the grants may cover

Applicants may seek funding to upgrade ambulances and other EMS equipment. The state materials frame the opportunity around documented equipment and service needs rather than a guaranteed payment for every applicant.

DPHHS says reviewers will consider the age, model and operational status of existing equipment; gaps in patient care; the level and type of EMS service provided; and the anticipated positive outcomes of the requested equipment.

How to apply

Applications must be submitted through the State of Montana’s Submittable portal under the listing titled “DPHHS EMS Equipment Grant Application.” The application window opened July 16, 2026, and closes August 15, 2026.

Before submitting, agencies should verify their rural-provider status, document the age and condition of existing equipment, describe patient-care gaps and explain the expected benefits of the proposed purchases. Vendor rules and what happens next

Agencies that receive equipment awards must purchase the equipment through a DPHHS-approved group purchasing organization or vendor.

The application guidance also says the department may adjust the amount provided under an agreement because of funding reductions, the governing budget, erroneous or improper payments, audit findings or failures in the awardee’s delivery of services. It says applicants will be notified if grant funding is denied or is no longer available.

The EMS opportunity is part of Montana’s larger Rural Health Transformation Program award. CMS lists Montana’s fiscal-year 2026 award at $233,509,359. The state’s announcement identifies the total financial-assistance award as $233,509,358.76 and says Montana could receive up to $1.2 billion over the five-year program, subject to federal conditions and future determinations.

For rural and frontier communities, local EMS agencies may provide emergency transport and initial care before patients reach larger facilities. The state describes the equipment grants as an effort to strengthen rural EMS, but the available records do not promise specific improvements in response times, coverage or patient outcomes.

The immediate deadline for qualifying rural EMS agencies is August 15, 2026. Agencies should confirm eligibility, prepare documentation of equipment and patient-care needs, and review the approved-vendor requirement before applying.
