NTSB Finds Overweight Flights and FAA Oversight Failures in Alaska Crash
The National Transportation Safety Board said July 30, 2026, that repeated overweight flights and inadequate Federal Aviation Administration oversight contributed to the February 6, 2025, crash of Bering Air Flight 445 near Nome, Alaska. The board also identified severe icing, inadequate airspeed management, degraded situation awareness and improper control inputs during an aerodynamic stall.
The Textron Aviation Cessna 208B was operating as a Part 135 scheduled charter flight. The pilot and nine passengers died. The NTSB‘s findings complete the investigation and do not create an immediate nationwide passenger restriction. They do, however, raise new questions about how the FAA monitors small commuter and air-taxi operators.
What investigators found
The NTSB determined that the probable cause was the pilot’s inadequate airspeed management, which the board attributed to workload and degraded situation awareness, along with improper control inputs leading up to and during the aerodynamic stall. Severe icing conditions complicated the flight.
The board also found that the airplane was operated above its maximum gross weight. That reduced the aircraft’s stall margin, leaving less room for error in conditions that already made the airplane more difficult to control. The NTSB treated the overweight operation as also causal, not as the sole explanation for the crash.
Investigators found that Bering Air’s safety management allowed routine flights that exceeded weight limitations. They also found a pattern of underreported aircraft weights on flight manifests, reducing the accuracy of information used for flight planning and weight-and-balance calculations.
Why FAA oversight is part of the finding
The NTSB said FAA oversight failed to identify and address the routine operation of overweight flights. The board also said the agency’s oversight did not sufficiently account for changes in Bering Air’s growth and operational complexity.
Those conclusions apply to the operator and oversight record examined in this investigation. They do not establish that all Part 135 carriers or air-taxi companies operate with the same weaknesses. Part 135 operators are subject to federal safety rules, but they generally operate under a different regulatory framework from large scheduled airlines.
What the NTSB wants changed
The NTSB recommended that the FAA require all Part 135 and Part 91K operators, except single-pilot and single-pilot-in-command operators, to add upset-prevention and recovery training, along with related checklists and procedures, to their training programs. The training is intended to help pilots recognize and recover from dangerous aircraft attitudes or loss-of-control situations.
The board also recommended that the FAA develop explicit surveillance criteria for Part 135 and Part 91K operators experiencing rapid growth, increased operational complexity, accidents, incidents or other changes that warrant increased oversight. The recommendation also calls for the agency to verify that inspector staffing is adequate and increase staffing where certificate oversight is insufficient.
The NTSB reiterated longer-running recommendations involving certificated dispatchers with joint responsibility for flight safety and operational control, flight-data recording and monitoring, and broader load-manifest and recordkeeping requirements. These are recommendations from the investigative board, not final FAA rules. The FAA has not adopted them simply because the NTSB issued them.
What happens next
The FAA is the next key decision-maker on whether and how to respond. Possible next steps include an agency response, proposed regulatory changes, revised inspection practices, additional training requirements or an implementation timeline. None of those changes is automatic under the NTSB’s report.
For passengers, the findings are most relevant to communities that rely on small commuter flights, charter services and air taxis. The report does not announce a new passenger ban or a broad grounding of Cessna 208B aircraft. Instead, it raises questions about whether federal oversight and operator procedures are identifying weight, training and operational-complexity risks early enough.
Travelers using small-aircraft services should watch for FAA action on surveillance, pilot training, flight-data monitoring and load documentation. Communities that depend on these flights will also be watching for safety improvements that preserve access to essential transportation.
